The "burned a CDR" and "downloaded music, books, or movies" one feel prejudicial - like the writer is trying to imply that makes you a pirate. Which is complete nonsesnse, I've burned CDs of non-music data that I created, and Project Gutenberg encourages you to download their free-as-in both freedom and as-in-beer books and read them, even share them.
I don't if I would would want to answer some of those questions unless it was anonymous (or you were the only one seeing the answers). I'd be concerned that, even if they said NO you will not be judged and/or later brought up on charges, I still would be.
"Feel prejudicial," yes, that's exactly how it feels to me as well. I'd feel extremely uncomfortable if I were on the jury and had to answer the questionnaire; I'd worry about how my answers would be used and fear someone try to force me to provide names. Sort of the way the police/courts do now with purported drug dealers (which explains why actual drug dealers often receive short sentences while their friends and family members -- who are not dealing and have no information to trade -- receive much long prison terms).
